What Is Diabetic Neuropathy?

The connection between diabetes and nerve damage comes down to circulation. When blood sugar levels remain elevated over time, they damage the small blood vessels responsible for delivering oxygen and nutrients to your peripheral nerves, particularly in the feet and hands. Without adequate blood flow reaching those distant tissues, the nerves begin to deteriorate and malfunction. The longer circulation remains compromised, the more significant the damage becomes.

This is a meaningful distinction from other types of neuropathy, because identifying circulation as the primary driver of the problem is what allows us to treat it effectively. Not all neuropathy works the same way, and a treatment approach that doesn’t account for the underlying cause is unlikely to produce lasting results.

Symptoms of Diabetic Neuropathy

Diabetic neuropathy most often affects the feet first, then the hands, and symptoms tend to progress gradually over time. Patients typically describe some combination of the following:

  • Tingling or a persistent “pins and needles” sensation
  • Numbness that may spread up the legs over time

One of the challenges with diabetic neuropathy is that the numbness it produces can mask pain that would otherwise serve as a warning sign. Foot injuries can go unnoticed, wounds can fail to heal properly, and the risk of serious complications increases over time. Treating the condition early, and addressing its root cause, matters.

Why Medication Doesn't Fix Diabetic Neuropathy

Gabapentin, Pregabalin (Lyrica), and Duloxetine are among the most commonly prescribed medications for diabetic neuropathy. They work by dampening nerve signal activity in the brain and spinal cord, reducing the volume on the pain signals your damaged nerves are sending.

What they don’t do is improve circulation to the affected nerves. They don’t repair the blood vessel damage that caused the problem. And they don’t stop the condition from progressing. For many patients, the medications provide partial relief early on, then become less effective as the underlying nerve damage continues to worsen.

The side effects add another layer of difficulty. Dizziness, fatigue, mental fog, weight gain, and balance problems are all common with these medications. For diabetic patients who already manage balance and mobility challenges, those side effects can significantly affect quality of life.
